About the Position
The Case Manager at Harris Health will facilitate a collaborative and interdisciplinary process in order to help patients and their families address and resolve the social, financial, and psychological problems related to the patient's health condition. You will identify options and advocate for services that will meet the patient's and their family's comprehensive needs with available resources to promote quality, cost-effective outcomes. In this role, you will also participate in various quality improvement activities while exemplifying professionalism in a customer-friendly environment.

Requirements
- Graduate from an accredited school of social work
- Master's Degree in Social Work
- Licensed Master Social Worker (LMSW)
- Licensed Clinical Social Worker (LCSW) preferred
- Related speciality certification within 2 years of hire
- Basic Life Support (BLS) from AHA approved program
- 2 years of experience as a social worker in healthcare preferred
- 1 year of experience in Case Management, Quality Management, or Discharge Planning preferred
- Spanish speaking preferred
